Location

Community based across Pembrokeshire, supporting individuals with learning disabilities, mental health needs or physical disabilities in their homes and local communities.

Pembroke, Pembroke Dock, Tenby, Saundersfoot, Kilgetty, Narberth, Maenclochog, Crymych and Newport, Fishguard, Goodwick, Letterston St Dogmaels, Haverfordwest, Milford Haven, Neyland, St Davids, Solva and surrounding areas.

Hours

We are very flexible and you can choose to work on your own schedule: mornings, evenings, school hours and weekends are not required.

Pay Rates

  • Weekdays – £13.45 per hour
  • Weekends – £14 per hour
  • Paid waiting time, travel time, mileage allowance

Job Purpose

To provide person centred support that enables individuals to live independently, participate in their community, and achieve personal outcomes in line with the Social Services and Well-being (Wales) Act 2014, CIW expectations and Quayside Care’s values.

Key Responsibilities

1. Community Inclusion & Independence

  • Support individuals to access community activities, appointments, education, volunteering and social opportunities
  • Encourage independence, confidence and self advocacy
  • Assist with travel training, budgeting, shopping and daily living skills

2. Personalised Support

  • Work in line with each person’s care and support plan
  • Promote choice, dignity and control in all aspects of support
  • Provide emotional support, companionship and encouragement

3. Practical Support

  • Assist with meal preparation, hydration and maintaining a safe home environment
  • Support with household tasks such as cleaning, laundry and organisation
  • Provide support with medication prompts or administration (following training and policy)

4. Communication & Relationships

  • Build positive, professional relationships with individuals, families and professionals
  • Communicate clearly and respectfully, adapting communication style where needed
  • Maintain confidentiality and comply with GDPR

5. Safeguarding & Professional Standards

  • Recognise and report safeguarding concerns in line with Wales Safeguarding Procedures
  • Uphold the Code of Professional Practice for Social Care (Wales)
  • Maintain professional boundaries at all times

6. Record Keeping & Compliance

  • Complete accurate, timely records using digital or paper systems
  • Follow all organisational policies, risk assessments and care plans
  • Report incidents, concerns or changes in need promptly

7. Health & Safety

  • Follow infection control procedures and lone working protocols
  • Use equipment safely and in line with training
  • Promote a safe environment for individuals and staff

8. Teamwork & Continuous Improvement

  • Attend training, supervision and team meetings
  • Contribute to a positive, supportive team culture
  • Participate in service development and quality improvement

Person Specification

Essential

  • Full UK driving licence with access to your own car
  • We are unable to offer sponsorship
  • Compassionate, reliable and committed to supporting independence
  • Good communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Willingness to travel within Pembrokeshire
  • Enhanced DBS check (Adults’ Barred List)
  • Right to work in the UK
  • Willingness to complete the All Wales Induction Framework (AWIF)

Desirable

  • QCF Level 2 or 3 in Health & Social Care
  • Experience supporting adults with learning disabilities, mental health needs or physical disabilities
  • Welsh language skills (valued but not essential)
