Children’s Home-Based Palliative Care Nurse (Band 6)

Central London Community Healthcare NHS Trust (CLCH) is recruiting a Band 6 Kaleidoscope Nurse to deliver high-quality care for children with complex and palliative needs in their homes across Kensington and Chelsea, Hammersmith and Fulham, and Westminster.

You will coordinate care, work with multisector teams, and provide on-call support to ensure continuity and safety for children and families.
