Job Description:
West London NHS Trust is seeking a Specialist Therapeutic Practitioner: Transition & Recovery to join the Transition Team on a part-time, permanent basis. The role supports people moving from inpatient to community care, using trauma-informed, strengths-based approaches.
You will deliver individual and group interventions, build therapeutic relationships, and work collaboratively across MDTs to reduce readmission and promote recovery and independence.
