Overview
In this role you join a multi-site GP team delivering population-based health and social care within an extended primary care framework. You will work with a multidisciplinary group to ensure timely patient access and high-quality care. You’ll handle a mix of in-surgery, home visits, and remote consultations, contributing to chronic disease management and health promotion. This opportunity supports professional development through MDT collaboration, research, and learning across specialties.

- Salary: 110,428 per year plus Recruitment and Retention Premia 16,881.15 pro rata

Responsibilities
- Make autonomous clinical decisions for presenting problems
- Consult with patients in surgery, at home, and via telephone/online channels
- Triage calls and visits; review and sign repeat prescriptions
- Screen for disease risk factors and craft care plans with patients
- Provide counselling and health education
- Admit/discharge patients and refer to other providers as appropriate
- Maintain high-quality clinical governance and policy adherence
- Support data collection, clinical audits, and health needs research
- Prescribe according to local formulary and standards
- Meet CPD requirements and engage in professional development
Key requirements
- Fully qualified GP with GMC registration
- Evidence of annual appraisal and revalidation
- General practice (VTS) trained
- Eligible for medical performers list
- Enhanced CRB check
- UK driving licence
- Current CPR certificate
- UK work permit if required
- Excellent communication
- Team working and interdisciplinary collaboration
