Job Description:
Hoop Social Work, in partnership with Cardiff Council, seeks an experienced Community Mental Health Social Worker to join the Cardiff CMHT. You will assess and plan care, review packages, and work with health professionals to deliver coordinated support for adults with complex mental health needs.
The role requires a recognised Social Work qualification, registration with Social Care Wales or Social Work England, and UK experience. DBS updated, driving licence, and references are required.
