Hybrid Social Care Practitioner: Assessments & Support

This is an exciting opportunity to join a dynamic, friendly and supportive Community Social Work team and support the team to complete assessments in line with the Care Act 2014 and arrange appropriate support. The post is currently 37 hours with some flexibility; we operate a hybrid model with requirements to work from the office and from home.
