About the Role

We are looking for an experienced Adult Social Care Brokerage Officer to support the effective sourcing and purchasing of care and support services for adults with assessed care needs.

The successful candidate will work closely with Social Workers, care providers, Finance, Contracts and other internal teams to arrange appropriate, high-quality and cost-effective care packages in line with authorised care plans and budgetary requirements.

Key Responsibilities

  • Source and arrange home care, residential, nursing, respite and other care placements in line with assessed needs and authorised care plans.
  • Identify suitable care providers based on an individual’s needs, preferences, availability and required level of support.
  • Liaise with care providers to confirm availability, suitability and start dates.
  • Negotiate fees and care package costs with providers, working within Council budgets and financial procedures.
  • Ensure care purchased provides value for money while meeting the individual’s assessed needs.
  • Manage care packages and placements, including planned and short-notice requirements.
  • Maintain accurate records of placements, vacancies, allocations and care packages on relevant systems.
  • Liaise closely with Social Workers, Team Managers, Finance, Contracts and external providers.
  • Ensure care packages are implemented promptly following authorisation.
  • Monitor and report on placement activity, performance, costs and any issues or concerns.
  • Support effective use of contracted and block-booked care arrangements.
  • Arrange appropriate transport and other services where required to meet individual needs.
  • Ensure customers receive clear information regarding their care package, service arrangements, fees and charges.
  • Support the timely discharge of customers from hospital by sourcing appropriate care and support where required.
  • Deal sensitively and professionally with providers, customers, families and internal stakeholders.
  • Contribute to the development and improvement of brokerage processes and services.

Essential Experience

  • Experience working within Adult Social Care in a brokerage, placements, commissioning, provider management or similar office-based role.
  • Experience of sourcing and arranging care packages or placements for adults.
  • Experience working with care providers and negotiating fees or packages.
  • Understanding of Adult Social Care processes, care plans and assessed needs.
  • Experience working within financial or budgetary constraints.
  • Strong data management, administration and record-keeping skills.
  • Ability to analyse information and use it to support decision‑making and negotiations.
  • Strong communication and stakeholder management skills.
