This permanent Band 6 role combines clinical leadership with direct patient care across the High Peak and North Dales catchment, focusing on non-pharmacological interventions to support independence and prevent hospital admission.
You will work within a MDT, travel across the service area, and cover 7 days per week with a 9am–5pm service
