Social Worker – Intermediate Care & Independence

North Yorkshire Council is seeking a Social Worker to join the Intermediate Care team in Skipton. You will work in people’s homes to prevent hospital admissions, discharge to Intermediate Care Beds when needed, and lead short-term high-impact interventions with individuals and families.

You will apply a strength-based approach, develop personalised care plans, assess eligibility under national standards, and maintain knowledge of the Care Act 2014.
