Job Description:
Wiltshire Council’s Continuing Healthcare Team is seeking a Level 3 Social Worker to assess adults for NHS Continuing Healthcare funding and undertake person-centred assessments. You will advise on CHC funding and help ensure appropriate support while promoting independence.
The role requires a qualified and registered Social Worker with Social Work England, a strong knowledge of safeguarding, health and social care legislation, and experience with vulnerable adults.
