Community Frailty Care Assistant

The Nhs Constitution is seeking a Health Care Assistant to support frail and housebound patients within the Armley PCN. You will undertake frailty reviews, maintain registers, assist with data collection, and work across GP practices to ensure patient needs are understood and met.
