Senior Practitioner, Adult Social Care – Discharge to Assess

Herefordshire Council is seeking a Senior Practitioner to join the Herefordshire Care Act Assessment Team in Hereford. The role involves a 37-hour week, managing a small caseload, leading practice, and supervising the team to improve performance and outcomes.

You will work with adults across health and social care, including safeguarding, enabling discharge planning and care planning via the Care Act pathway.
