Job Description:
Connect2Hampshire is seeking an Intermediate Care Social Worker for a 3-month temporary assignment. You will support hospital discharge planning for adults with mental health needs, working with families and partner agencies to enable safe community living. The role is hybrid, with meetings in Winchester once a week.
Applicants must have MH and discharge process experience and a strong strengths-based approach to social care. This agency role is exclusive to Connect2Hampshire.
