What you’ll be doing

Our Adult Social Care Division is committed to enabling adults with social care needs to:

  • Have control over their lives.
  • Live safe, healthy, and fulfilled lives.

  • Access the support they need to achieve the level of independence they choose.

As a key member of our multi-disciplinary team, you will manage complex cases, support colleagues, and contribute to the development of future services. You will be responsible for:

  • Receiving and coordinating complex referrals.
  • Carers assessments
  • Approved Mental Health Professional (AMHP) assessments
  • Best Interests Assessor (BIA) assessments
  • Developing and reviewing person-centred support plans.
  • Coaching, mentoring, and supporting Social Care Assessors.
  • Managing risk and undertaking crisis intervention where required.
  • Promoting advocacy, user involvement, and strengths-based practice

What we expect from you

We are looking for a qualified and motivated Social Worker who has:

  • Significant post-qualification experience in social care or health settings.
  • Experience of mentoring, coaching, and supporting colleagues.
  • Strong assessment, risk management, and decision-making skills.
  • Experience of partnership and multi-agency working.
  • The ability to undertake AMHP and/or BIA assessments.
  • A commitment to person-centred practice, advocacy, and positive outcomes.
  • Excellent organisational skills and the ability to manage competing priorities.
  • Confidence representing the organisation in court or formal settings.
  • Social Work qualification (BSc, DipSW, CQSW, or equivalent recognised by Social Work England).
  • Current Social Work England registration.
  • Commitment to ongoing professional development.
  • Ability to travel to visit individuals and families in their homes.
  • Flexibility to work across Adult Social Care pathways.
