Community Assessment Officer — Early Help & Independence

Salford Council’s Adult Social Care Contact Team is seeking a Community Assessment Officer to conduct strengths-based conversations, complete early help and initial assessments, and visit communities as needed to connect people with local support.

You will work with health professionals, social workers, housing providers and VCSE partners; the role emphasizes independence, wellbeing, and building on people’s strengths with strong communication and organisational skills.
