Medacs Healthcare is seeking experienced CHC Nurse Assessors for Cheshire and Merseyside ICB. The position is fully remote with a mixed pattern of duties including caseload management and assessment-based decisions, working within a Band 6 framework.
You will assess care needs, coordinate packages, and liaise with NHS services, local authorities and care providers to ensure appropriate care and support. Flexible Mon-Fri hours, remote working, and competitive pay accompany the role.
