Rotherham Doncaster and South Humber NHS Foundation Trust invites applications for a Mental Health Practitioner to join the Hospital Liaison Team, providing urgent responses to Mental Health referrals from TRFT and undertaking bio-psychosocial assessments for safe discharge planning.
You will work with TRFT staff to develop knowledge of Mental Health and support TRFT staff through training and promoting patient-centred care, with a flexible 24/7 duty rota and full-time applications welcome.
