Job Description:
Calderdale and Huddersfield NHS Foundation Trust is recruiting a qualified registered nurse to join the Acute Frailty service. You will start assessments from ED or ward, working with consultants, geriatricians, therapists and social services to deliver holistic CGA.
You will act as a clinical role model, manage a specialist caseload and contribute to service development within a supportive, multidisciplinary team.
