This is a unique opportunity to play a pivotal role in delivering six large-scale Community Appointment Days between January and March 2027, helping people access timely, person-centred support through a “Whole Person, Whole System” approach focused on “What Matters to You”, shared decision-making and coordinated community care.

About the Role

Reporting to the Partnership Director and working alongside NHS, local authority, community and voluntary sector partners, you will provide operational leadership for the planning, mobilisation and readiness of the CAD programme.

You will coordinate multiple workstreams to ensure each event is safe, effective, clinically governed and fully prepared for delivery. Working across organisational boundaries, you’ll bring together people, processes, venues, workforce plans and governance arrangements to create seamless patient experiences and measurable improvements in care.

Key Responsibilities

  • Lead the mobilisation, planning and readiness assurance for six Community Appointment Days across East Sussex.
  • Develop and manage integrated programme plans, milestones, risks, dependencies and governance processes.
  • Coordinate clinical, operational, digital, estates, workforce and partner workstreams.
  • Ensure robust safety, safeguarding, escalation and operational procedures are in place.
  • Work with clinicians and stakeholders to design patient-centred pathways and inclusive access arrangements.
  • Secure and coordinate community venues, workforce requirements and event-day operations.
  • Build strong partnerships with NHS organisations, primary care, local authorities, Public Health teams and VCSE partners.
  • Drive programme performance, evaluation, reporting and benefits realisation.
  • Produce a comprehensive programme closure and handover report by March 2027.

We are looking for a highly organised and collaborative programme professional who can confidently lead complex, time-critical initiatives within healthcare, social care or another regulated environment.

You will bring:

Significant programme or project management experience within complex organisations

Strong operational planning, governance and readiness assurance expertise

Experience managing multiple stakeholders across organisational boundaries

Excellent risk management, reporting and problem-solving skills

A track record of delivering change and service improvement programmes

Knowledge of healthcare governance, patient safety and quality standards

Outstanding communication, facilitation and relationship-building abilities

  • NHS, community healthcare or integrated care programmes
  • MSK pathways, triage, rehabilitation or service redesign
  • Local authority, Public Health or VCSE partnership working
  • Benefits realisation, evaluation and quality improvement methodologies

Why Join Us?

This is an opportunity to make a lasting impact on healthcare delivery across East Sussex by helping create innovative, accessible community services that improve patient outcomes and experiences.

You’ll work with a passionate group of clinical, operational and community leaders dedicated to delivering meaningful change and reducing waiting lists through collaborative, person-centred care.
