An exciting opportunity has arisen to join Derbyshire Community Health Services’ Specialist Tier 3 Weight Management Service as a Weight Management Adviser.
We are looking for an enthusiastic, motivated and compassionate individual with experience of supporting people to make sustainable lifestyle and behavioural changes. You will work as part of a specialist multidisciplinary team helping adults with complex obesity to improve their health and wellbeing through personalised, evidence-based interventions.
Our Tier 3 service takes a psychologically informed approach to weight management, recognising the biological, psychological and social factors that contribute to obesity. You will support patients through individual and group-based interventions, helping them develop realistic goals, build confidence and make positive long-term changes to their eating, physical activity and health behaviours.
The role offers the opportunity to work closely with Clinical Psychologists, Consultant Physicians, Dietitians and other specialist colleagues within a supportive and innovative team environment. You will also contribute to the development of patient resources, service improvements and digital approaches to patient care.
We welcome applications from individuals with experience or qualifications in areas such as health coaching, physical activity, nutrition, public health, behaviour change or related disciplines.
Main duties of the job
The successful candidate will:
- Deliver evidence-based weight management assessments, education, advice and ongoing support to adults with complex obesity.
- Manage a caseload of patients receiving specialist Tier 3 weight management support, providing personalised coaching and behaviour change interventions.
- Work collaboratively with the wider multidisciplinary team, ensuring patients access specialist dietetic, psychological and medical support when appropriate.
- Facilitate and support both face-to-face and virtual group education sessions.
- Support patients to set realistic goals, monitor progress and identify barriers to achieving sustainable lifestyle change.
- Collect, record and interpret outcome measures in line with service requirements and quality standards.
- Undertake baseline assessments of physical health, medications, obesity-related conditions and lifestyle factors in accordance with service protocols.
- Contribute to service development, quality improvement initiatives and the creation of patient resources.
- Maintain accurate clinical records and work in accordance with professional, organisational and information governance standards.

The Specialist Tier 3 Weight Management Service is a well-established, multidisciplinary service supporting adults with complex obesity across Derbyshire. Our team includes Clinical Psychologists, Consultant Physicians, Dietitians, Weight Management Advisers and administrative colleagues who work together to provide personalised, evidence-based care.
The service is continually evolving to meet the needs of patients and incorporates innovative approaches including digital delivery, personalised care planning and close MDT working. We value new ideas and actively encourage staff to contribute to service development and quality improvement.

Person specification
Education, Qualifications and Training
- Level 3 exercise referral qualification (or equivalent) such as YMCA FIT Diploma in exercise referral or Wright Foundation exercise referral or willing to train.
- Level 3 nutrition and weight management qualification (or equivalent) or willing to train
- Training in motivational interviewing/behaviour change/counselling or relevant experience
- Level 4 qualification in obesity management or equivalent experience
Experience and Knowledge Required
- Experience of working with obese individuals attempting to lose weight
- An understanding of how to support patients through change management and goal setting
- Knowledge of the factors/barriers individuals face when attempting weight loss, including morbidly obese individuals
- Knowledge of current healthy lifestyle messages and physical activity recommendations, research evidence, good practice etc.
- Experience of working with individuals in order to facilitate behaviour change
- Experience of working with individuals over long time periods in order to aid weight reduction/maintenance.
- Experience of working with individuals with a range of health issues.
