Requirements

• Qualified Social Workerwith a recognisedSocial Work qualification.
• Registered with SocialWork England.
• Experience working withinAdult Social Care, HospitalDischarge or CommunityTeams.
• Good knowledge ofthe Care Act2014, Mental CapacityAct and safeguardinglegislation.
• Excellent communication,assessment and careplanning skills.
• Full UK drivinglicence and accessto a vehicleis desirable.
