About the role;

Are you a Qualified Social Worker with experience in adult mental health and Community Mental Health Teams (CMHT) ?

We have an excellent opportunity for a dedicated Social Worker to join an established multidisciplinary team in Dorset, supporting adults with complex mental health needs. You will be responsible for a varied caseload, undertaking statutory Care Act duties, assessments and reviews, while working collaboratively with health and social care professionals.

The role will also involve regular community and home visits across Dorset, so candidates must be comfortable working within the community.

Key Responsibilities:

• Complete strengths-basedassessments in linewith the CareAct 2014.
• Support safeand timely hospitaldischarges and preventunnecessary admissions.
• Develop and reviewperson-centred careand supportplans.
• Work closely withmultidisciplinary teams, including NHS colleaguesand community services.
• Undertake Mental CapacityAssessments and BestInterest decisions whereappropriate.
• Manage a variedcaseload while maintainingaccurateand up-to-date caserecords.
• Promote independence andenable service usersto achieve thebest possible

Requirements:

• Qualified Social Worker with current Social Work England registration.
• Previous experience within Community Mental Health, Adult Social Care or CMHT services.
• Strong working knowledge of the Care Act, Mental Health Act and Mental Capacity Act.
• Proven experience managing complex cases, safeguarding and risk.
• Excellent assessment, report-writing and communication skills.
• Ability to work effectively within a multidisciplinary team.
• Ability and willingness to undertake community and home visits.
• A confident, professional and person-centred approach to supporting adults with complex needs.
