Outreach Navigator for Complex Needs — Social Care

The Bridge Project in Bradford delivers services supporting people with multiple needs across housing, health, and social support. The Navigator role works in outreach to help individuals access the services they need to build resilience, confidence and personal assets.

Supported by a Senior and Service Manager, the role requires Social Care Level 3 or equivalent and a driving licence. It is a full-time, permanent position with a strong commitment to safeguarding and wellbeing.
