Join our friendly and supportive Integrated Care Teams based at Havant Health Centre, St Mary’s campus, Waterlooville and Petersfield providing high-quality community-based care to patients registered with local GP practices. Working as part of a multi-disciplinary team, you will support registered practitioners by undertaking delegated nursing tasks and delivering person-centred care in patients’ homes and community settings., To provide a supporting role to registered practitioners in the Integrated Community Team in the delivery of nursing care respecting patients’ privacy and dignity at all times. To act as key worker for patients under the direction of the registered practitioners. To assist in planning, delivering and evaluating programmes of care prescribed by the registered practitioners, regularly liaising at the beginning and throughout each shift. To observe, monitor and record patient activity and health status as appropriate, communicating any changes in their condition to the registered practitioners. To be responsible for progressing patient’s treatment plans within guidelines set by registered practitioners and regularly contributing to the review of each patient’s progress within the team.
