Palliative Home Care Assistant

Trinity Hospice provides compassionate, person-centred care for patients in the last year of life, delivered in patients’ homes by a dedicated Community Nursing Team. The Health Care Assistant will support admissions, discharges and ongoing care packages, and help with daily living tasks while ensuring dignity and comfort.
