Central London Community Health Trust is seeking an experienced Band 8b Clinical Operations Manager to provide operational and system leadership for the Barnet Integrated Transfer of Care Hub and Hospital at Home service.

You will lead multidisciplinary teams across CLCH, Barnet Hospital and Barnet Adult Social Care, drive improvements in patient flow and discharge, and work with system partners to deliver safe, high-quality care.
