Job summary

We are seeking a dedicated Clinical Pharmacist Prescriber to join our GP surgery at Addison Road Medical Practice. The ideal candidate will play a crucial role in ensuring optimal patient care through the safe and effective use of medications. This position requires the ability to work collaboratively within a multidisciplinary team to enhance patient outcomes.

Main Responsibilities

Key duties will include:

  • Prescribing, online, F2F and telephone clinical triage, management of minor illness and LTC advice.
  • Delivering safe, clinical decisions and expert care as well as working collaboratively with all team members of a multi professional team and driving innovation, modernisation and high levels of staff and patient satisfaction.

Main duties of the job

Same-Day Demand Management

Work within our Triage Hub to assess and manage same-day patient requests, ensuring safe and effective prescribing

Use clinical expertise to support and streamline workload

Providing administrative support in managing administrative related demand

Structured Medication Reviews

Conduct comprehensive medication reviews

Attend and refer patients to multidisciplinary case conferences

Long-Term Condition Management

Manage own case load, running long-term condition clinics, within professional scope which could include but is not limited to hypertension, diabetes, asthma, and COPD. Optimising medication in line with guidance and ensuring appropriate follow up

Collaborate with the wider clinical team to provide proactive, patient-centred care

Risk Stratification

Responsibility for management of risk stratification tools on behalf of the practice using Eclipse Live where necessary as well as other digital tools such as Ardens and Primary Care IT

Repeat prescribing

Manage the repeat prescribing reauthorisation process

Support the practices vision of working towards a fully managed pharmacist service delivery for repeat prescribing

Signing prescriptions within professional scope

Addison Road Medical Practice (ARMP) is a large teaching practice based in a purpose-built building in Walthamstow, North East London (NEL). The practice serves 18 000 patients and is part of a wider group of 7 GP Practices spread across NEL and SEL serving 75000 residents. The Ideal Candidate:

  • Undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
  • Independent prescribing qualification desirable
  • Ability to apply effective clinical judgement in the prioritisation and decision‑making aspects of the role.
  • Previous general practice experience is a requirement
  • Previous experience in Triage is desirable
