Overview
In this role you join the North Westminster Community Mental Health Hub to support accessible, high-quality mental health care in a community setting. You will conduct assessments, manage referrals, and deliver brief interventions within integrated care pathways, collaborating with GP practices and MDTs. You’ll contribute to safeguarding, training, and service improvement, helping to fast-track appropriate care and maintain standards. Responsibilities
- Receive and triage GP referrals and conduct psychosocial assessments to identify needs, risks, and care pathways
- Act as liaison for GP practices, supporting patients with complex presentations and comorbidities
- Screen referrals for suitability for primary care management within thresholds and resources
- Facilitate onward referrals and rapid access to Hub pathways, specialist teams, or external agencies
- Ensure early identification of severe mental illness or high risk and escalate safeguarding as needed
- Refer common mental health problems to psychological therapies or community resources
- Engage with GPs and practice nurses to support integrated care
- Deliver high standards of care using psychosocial, problem-solving, and evaluative approaches
- Use evidence-based therapeutic techniques tailored to individual needs
- Participate in Hub duty rota and contribute to triage decisions
- Demonstrate cultural competence and respect for diverse backgrounds
- Engage in regular clinical supervision to support safe practice
- Support Hub depot clinic operations as required
Key requirements
- Current Professional Qualification in Mental Health Nursing
- UK NMC registration
- Post-registration CPD and training
- Experience in a multidisciplinary mental health team with acute/severe and enduring conditions
- Extensive post-registration experience
- Excellent verbal and written communication; risk assessment and management skills
- Knowledge of psychosis, legislation, and care for diverse groups
- Desirable: psychosocial interventions, Thorn Therapy/Work training, leadership potential
- Desirable: experience with homelessness, crisis work, supervision, audit/research
- Disclosure and Barring Service check requirements
