Overview
In this role you will join the medicines management team at Ashbourne Medical Practice to lead safe, patient-centred medication management. You will perform structured medication reviews, support staff with prescriptions, and help integrate general practice with wider healthcare teams. The role offers hybrid working and the chance to improve care quality for an expanding patient population. You will contribute to antimicrobial stewardship, QOF, and service development within a supportive, teaching-practice setting.

Responsibilities
- Lead structured medication reviews for patients with polypharmacy and multiple comorbidities
- Provide expert medicines advice to general practice staff on prescriptions
- Oversee AI/software-driven workflows (Welby Innovate) for safe medication monitoring
- Prescribe or train to prescribe within the practice team
- Support LTC, frailty, learning disabilities, and autism patient reviews
- Drive medicines optimisation and quality improvement, contribute to QOF and enhanced services
- Lead integration with wider PCN, community, and hospital pharmacy
- Participate in audits, safety reviews, and education for clinical staff
- Contribute to safer prescribing systems and medicines reconciliation on transfer of care
- Support repeat prescription system and acute requests
- Promote self-care and reduce waste through medication management
Key requirements
- MPharm and GPhC registration
- Independent prescriber (IP)
- UK driving license
- UK work permit if required
- Enhanced DBS check
- Annual appraisal and revalidation (when appropriate)
