Remote Health and Human Service Case Manager I

Department of Health and Social Services (DHSS) in Delaware seeks an entry‑level Health/Human Service Case Manager I. You will determine eligibility for SNAP, Medicaid, CHIP, childcare, and GA, and refer for benefits not provided by the DSS. You will conduct counseling and case management with vulnerable, low‑income residents.

You will work with SNAP vendors, prepare histories, and coordinate with hospitals and community resources to implement service plans and support clients effectively.
