Autonomous Primary Care Mental Health Clinician

South West Yorkshire Partnership NHS Trust in Brighouse is seeking a Band 6 Primary Care Mental Health Practitioner to deliver specialist mental health support within Calderdale’s PCN network. You will conduct mental health assessments, determine needs and guide patients to the most appropriate service, with brief therapeutic work where suitable.
