Join our friendly and supportive Integrated Care Teams based at Havant Health Centre, St Mary’s campus, Waterlooville and Petersfield providing high-quality community-based care to patients registered with local GP practices. Working as part of a multi-disciplinary team, you will support registered practitioners by undertaking delegated nursing tasks and delivering person-centred care in patients’ homes and community settings.
This rewarding role involves home visiting and lone working, helping to prevent unnecessary hospital admissions, support early discharge from hospital, and promote independence for patients living with long-term conditions and older adults with mental health needs. You will play a key role in improving patient outcomes and enhancing quality of life while working within a collaborative and compassionate healthcare team.
This is an excellent opportunity for a caring, motivated individual who enjoys working independently and making a real difference in their local community. Due to the community-based nature of this role, a full UK driving licence and access to a vehicle are essential to enable travel between patient visits across the locality.
