What you’ll do
As a Hospital to Home Care Assistant, you’ll support people following their discharge from hospital, helping them settle safely back into their own homes and regain their independence.
You will:
- Support people to remain safe and independent at home
- Provide personal care with dignity and respect
- Assist with medication, mobility and mealtimes
- Offer companionship and reassurance following a hospital stay
- Help people rebuild their confidence and daily routines
- Work closely with your local team to provide reliable, person-centred care
Qualifications
What we’re looking for
No previous care experience? No problem. Full training and support will be provided.
We’re looking for people who are:
- Naturally caring, compassionate and patient
- Reliable and committed
- Comfortable supporting people in their own homes
- Keen to learn and develop within care
- Able to work either 7am-3pm or 3pm-10pm shifts
