Job Description:
Connect2Hampshire is recruiting a Adults Hospital Discharge Social Worker (Locum) for Hampshire County Council, based at Queen Alexander Hospital, Portsmouth. Four days on site and one day from home, 37 hours a week.
The role involves carrying out Care Act and Mental Capacity assessments, making best interests decisions, planning safe discharges from acute settings and coordinating with health, nursing and therapy colleagues.
