Lead Nurse – NHS@Home (Band 6)

Bristol NHS Foundation Trust is seeking a Band 6 Sister/Charge Nurse to lead NHS@Home operations from the Southmead Site in Bristol. You will oversee patient assessments, coordinate MDT referrals, and support development of new clinical pathways to enhance in-home care for acutely unwell patients.

The role requires an acute clinical background, autonomous decision-making, and ability to work with multidisciplinary teams. Flexibility for community visits and a dynamic care setting is expected.
