Home Health Care Assistant – Community Care & Growth

Buckinghamshire Healthcare NHS Trust is seeking Healthcare Assistants to join the Community Nursing Service, delivering direct patient care at home and within community settings. You will support long-term condition reviews, phlebotomy, continence care and other delegated tasks, working alongside Community Nurses to help patients stay independent and reduce hospital admissions.
