Job Description:
BCUHB North Wales is seeking a skilled Mental Practitioner to join an established Community Mental Health Team. You will work within an integrated, multi-disciplinary service focusing on individuals with severe and enduring mental health problems, delivering care co-ordination and proactive support.
You will contribute to Care and Treatment Plans, ensure adherence to the Mental Health Measure (Wales) 2010, and conduct risk assessments with clear documentation.
