Community Nurse Practitioner Learning Disability Lead

Hertfordshire Partnership University NHS Foundation Trust is seeking a registered nurse with learning disability expertise to assess and treat people in hospital, home and community settings, delivering person-centred care and promoting recovery.

You will lead care coordination, supervise staff, complete risk assessments and care plans, and work with service users, families and the multidisciplinary team to improve physical, mental and emotional health. UK registration with the NMC is essential.
