Caritas Recruitment is currently recruiting for an experienced Social Worker to join a well-established Mental Health Team based at Fountain Way Hospital in Salisbury.

This is an excellent opportunity to work within a specialist adult mental health service supporting adults with severe and enduring mental health conditions. You’ll be joining a supportive and experienced team that places a strong emphasis on wellbeing, collaboration, professional development and person-centred practice.

The Role

• Assess, coordinate and arrange care for adults with mental health needs.




• Work across both community and hospital settings.




• Complete strengths-based assessments and care planning.




• Support recovery, independence and positive outcomes.




• Work collaboratively with individuals, families and partner agencies.




• Promote choice, control and person-centred decision making.

Requirements

• Social Work England registration.




• Previous experience within Adult Mental Health services.




• Strong assessment and care coordination skills.




• Experience working with adults with complex mental health needs.




• Ability to work flexibly across community and hospital settings.
