Leaving Care Social Worker — Shape Young Futures (Hybrid)

Hoop Social Work is supporting Caerphilly County Borough Council to recruit a Leaving Care – Social Worker. The team are based in Ystrad Mynach with hybrid working arrangements.

You will undertake statutory social work duties for leaving care, complete assessments and care planning, and safeguard young people in collaboration with partners to secure safe outcomes. Applicants must hold a social work qualification and registration with Social Care Wales or Social Work England.
