Band 6 Community Nurse/Case Manager – Integrated Home Care

Buckinghamshire Healthcare NHS Trust is recruiting a Band 6 Community Nurse/Case Manager for the Aylesbury area. The post focuses on proactive community care, caseload management, prevention of admission, reablement and end of life care, with discharge planning in partnership with care partners.

You will work within a 7 days a week service and shift patterns; lead a team, deliver high standards of care across locations, coordinate with other agencies and manage care plans.
