S is a friendly and approachable individual with complex health and care
needs following a stroke in 2020 and other long-term medical conditions.
She is a wheelchair user and is supported by a structured care package
for day-to-day personal care and mobility.
S enjoys getting out and about, spending time with family and taking part
in activities that help her feel engaged and connected to her community.
She values someone who will listen, respect her choices and tailor
support to suit her individual needs.
This role is focused on coordination, organisation and facilitation of
services and activities, helping S to access medical appointments,
physiotherapy, community activities and social opportunities in a timely
and consistent way.
S lives at home and is supported closely by her family, including her two
daughters, so it is important that the right person can work collaboratively
with both S and those involved in her care.
