Recovery Practitioner — Community Outreach (Substance Misuse)

Via is recruiting a Recovery Practitioner for the Cheshire West and Chester Service to deliver substance misuse treatment and recovery support. The role involves daily travel to rural communities, service hubs, and people’s homes as part of a community-based approach.

The successful candidate will undertake assessments, home visits and outreach, manage a caseload, and deliver harm reduction, psychosocial interventions, and overdose prevention advice, working with multi-disciplinary teams.
