Discharge Navigator for Hospital Care Teams

The EIT Discharge Coordinator role is based in the Emergency Department, Same Day Emergency Care (SDEC) and short stay wards at Cambridge University Hospitals NHS Foundation Trust. You will coordinate discharges for adult patients to streamline the hospital journey.

You will track patient progress, develop robust discharge plans and work with the MDT to ensure timely, safe community or hospital-based pathways.
