Medical Social Worker – Care Navigator & Advocate

JOBSTUDIO PTE. LTD. is hiring Medical Social Workers (MSW) for acute and community settings in the UK. You will conduct intake assessments, participate in case conferences with multidisciplinary teams, and support the development and review of ICPs.

Responsibilities include monitoring client progress, visiting clients at home or in hospital as needed, guiding families on financial assistance schemes and community resources, and performing other duties as assigned by the reporting manager.
