Lincolnshire County Council is seeking a Wellbeing Personal Adviser in Sleaford to support care-experienced young people aged 16-25. This permanent role focuses on tailored guidance, advocacy, and transitions to independent adulthood, with travel across Lincolnshire.
You’ll work in a relationship-based capacity with multi-agency teams, using Signs of Safety and Valuing Care frameworks to develop Pathway Plans covering accommodation, health, education, finances, and life skills.
