Wellbeing & Recovery Practitioner (Desistance Focus)

The Forward Trust in Hull and East Yorkshire is seeking a More Than My Past Practitioner to join the Connections Team at HMP Millsike. This full-time role supports individuals in custody to build healthier lifestyles and resilience that can be sustained on release.

You will deliver 1:1 support, group workshops and health promotion activities, collaborate with prison staff and external partners, and maintain care plans and records to track progress.
