Lead Nurse, Integrated Community Care (Band 7)

Whittington Health NHS Trust in partnership with UCLH is recruiting a Sister/Charge Nurse (Band 7) for the UCLH@HOME service. The post holder will provide nurse-led care to patients managed in a community setting, working with families, carers and a multi-professional team to develop care plans and promote optimal outcomes.

The role emphasises an integrated care pathway across acute and community settings, with support for autonomous practice and scope expansion.
