Job Description:
Sussex Community NHS Foundation Trust is seeking a Heart Failure Specialist Nurse to join a well-established community team on a part-time basis (22.5 hours per week), across the High Weald, Lewes and Havens, with potential to work across the SCFT footprint.
The role focuses on case management of newly diagnosed and long-standing heart failure patients, aims to optimise guideline therapies, prevent admissions, and support palliative care when appropriate.
