Reentry Peer Support Specialist — Prison to Community

The Forward Trust is recruiting a Hertfordshire RECONNECT Peer Support Worker to help people leaving prison access vital health and wellbeing support. The role is based in Hertfordshire and involves delivering a person-centred service in custody and in the community, with assessments and tailored care planning to aid successful reintegration.

You will manage a varied caseload, coordinate with community services, and lead the prison peer support programme.
